안녕하세요,  영종도 하늘도시 르네의원 대표원장 문성호 인사드립니다! ​ 영종도에서 르네의원을 개원하고, 피부 진료를 도움 드린지 어언 1년 4개월이 되어가고 있습니다 ^^ ​ 그리고 첫 인사를 드리는 지금 ​ ​ “그냥 뻔한 광고겠지..” ​ ​ 라고 생각하셨겠지만? ​ 이 글은 저희 르네의원을 찾아달라는 뻔한 홍보글이 아닙니다. ​ 제가 왜 르네의원을 개원했는지, 그리고 르네의원에서 어떤 마음으로 환자분들을 마주하고 있는지 ​ 저의 진심과 진료 철학을 전해드리기 위해 작성하는 글입니다. ​ 소중한 여러분의 피부를 믿고 맡길 수 있으려면 무엇보다 ‘신뢰’가 기반이 되어야 하기에.. ​ 저의 철학과 르네의원만의 원칙에 대해 솔직히 말씀드리려 합니다. ​ 이 글을 읽는 여러분들을 위해 정성껏 작성했으니 끝까지 읽어주세요 :) 저도 환자였습니다. 저는 사실 어린 시절, 피부가 그리 좋지 않았습니다 ^^; ​ 덥수룩해 보이는 수염 자국은 고사하고 여드름 흉터에다, 선크림도 제대로 바르지 않아 거뭇거뭇한 잡티와 색소도 많았죠. ​ 10년 전 사진이지만.. 지금 봐도 참 참담하다 싶습니다 ㅎㅎ; ​ 때문에 컴플렉스를 가지고 정말 여러 병원을 다녀봤습니다. ​ 1년 동안 다닌 피부과만 10군데가 넘을 정도였으니까요. ​ 하지만.. 제게 맞는 방법으로, 책임감 있게 시술해 주는 병원은 찾기가 어려웠어요. ​ 내 피부 특성에 대해 얘기해주기는 커녕 ​ 그저 공장처럼 남들과 똑같은 기계로, 똑같은 세기로 치료를 받았습니다. 그리고 제가 의사가 되고 처음 피부 진료를 시작했을 때도 그랬습니다. ​ 서울 강남, 명동 한복판의 미용 의원에서 일했기에.. 더욱 회의감과 매너리즘에 빠졌었죠. ​ 하루 200명 이상 진료를 도움 드려야 했던 대형화된 공장식 병원과 치료의 한계를 뼈저리게 느꼈습니다. 안녕하세요. 하늘도시 르네의원 대표원장 문성호입니다. V라인, 아마 이 글을 읽고 계신 많은 분들께서 꿈꾸는 얼굴라인일 겁니다. 그리고 얼굴을 갸름해 보이게 해주는 데 있어 가장 간단하고, 합리적인 방법이라고 한다면.. 저는 사각턱 보톡스를 꼽고 싶습니다. 수술은 물론이고 다른 리프팅 시술에 비해서도 굉장히 간편한 편이니까요. 대략 10여년 전까지만 해도 약제 값이 정말 비쌌지만, 지금은 국내에서도 많이 생산되면서 금액적으로도 저렴해졌고요. 때문에 다른 시술은 안 받아봤거나 잘 모르더라도 보톡스만큼은 알고 계신 분들이 많을 정도로, 피부 진료에 있어 가장 대중화된 시술인 듯한데요. 다만 단순히 'V라인 만들어주는 시술', '얼굴 갸름해지게 해주는 시술'이라고 하기에는 그 원리와 기전에 조금 부합하지 않는 면이 있긴 합니다. 그래서 오늘은 이 사각턱,  영종도보톡스에 대해 정확히 짚고 넘어가려 합니다. 사각턱 보톡스가 뭔가요? 사각턱 보톡스는 턱에 있는 교근이라는 근육에 보톡스를 주입하는 시술입니다. (교근은 귀 아래, 턱 끝에 손을 대신 뒤 치아를 앙~ 물어보시면 느낄 수 있으실 겁니다. 치아를 깨물 때마다 불룩불룩 튀어나오는 느낌이 든다면 그것이 교근입니다.) 그럼 근육의 크기가 줄어들고, 얼굴이 갸름해지는 효과를 기대할 수 있죠. 근육이 줄어든다고? 보통 일반적인 영종도보톡스는 '표정 주름'의 예방과 완화를 위해 사용됩니다. 눈썹을 들어올리거나, 미간을 찌푸리거나, 눈웃음을 짓거나 할 때 같이 움직이는 얼굴의 약한 주름들을 움직이지 못하게 막아주지요. 이런 주름 보톡스의 경우 일반적으로 시술 2주 정도가 지나면 점차 주름이 완화되고, 같은 표정을 지어도 주름이 잘 생기지 않는 것을 확인할 수 있습니다. 그런데 사각턱 보톡스는 이와는 작용 기전이 조금 다릅니다. '근육을 움직이지 못하게' 하는 것은 동일하지만, '크기'를 줄이는 데 목적이 있으니까요. 더 구체적으로 설명드리기 위해 예를 들어보자면.. 다리를 크게 다쳐 통 깁스를 1-2개월 정도 하면 그쪽 다리 굵기가 확연히 가늘어진 모습이 나타납니다. 깁스를 하는 동안 다리 근육을 사용하지 못해 점차 빠지는 것이죠. 의학적으로는 '불사용 근위축'이라고 이야기합니다. 사각턱 보톡스는 이와 마찬가지로 의도적으로 근육의 움직임을 막아 볼륨을 줄이는 원리를 이용합니다. 그래서 가시적인 효과를 보려면 대개 3-4주 정도는 걸리기도 하죠. 시간이 지남에 따라 보톡스의 효과가 떨어지고, 근육의 크기도 점차 돌아오고요. 유지기간은 일반적으로 6개월 정도라고 말씀드립니다. 효과가 없다? 진료실에서 환자분들의 이야기를 듣다 보면, 몇 차례 영종도보톡스를 받았음에도 별다른 효과가 없었다고 말씀하시는 분들도 계십니다. 사실 체감상으로는 꽤 적지 않은 분들이 같은 고민을 가지고 계실 것 같은데요. 이유는 여러 가지가 있을 수 있겠으나, 크게 2가지로 정리해보겠습니다. 첫 번째는 내성입니다. 조금 복잡한 내용이지만 간략하게나마 말씀드리자면, 보톡스는 '독소'를 이용합니다. (tox = toxin) 이 독소가 근육을 움직이게 만드는 신경전달물질의 분비를 막아 주름이 완화되고 크기가 작아지는 것이죠. *간혹 근육을 마비시킨다고 말하기도 하던데, 비슷하긴 하나 저는 딱 적절한 표현은 아니라고 생각합니다. 어쨌든, 우리 몸은 어떤 독이 반복적으로 들어오면 '항체'를 만들게 됩니다. 이 항체가 생기면 독소는 제 역할을 하지 못하게 되니 결과적으로 보톡스의 효과가 떨어지거나, 반응이 나타나지 않게 되는 것이죠. 두 번째는 오진입니다. 얼굴이 넓어보이는 데에는 다양한 이유가 있습니다. 원래 골격 자체가 그러하신 분들도 계시고, 혹은 교근 아래에 있는 귀밑샘이나 침샘이 비대해져 있을 가능성도 있죠. 뼈는 수술적인 방법이 없다면 그 사이즈 자체를 줄일 수는 없지만, 귀밑샘이나 침샘은 마찬가지로 보톡스를 활용하면 좋은 결과를 기대해볼 수 있습니다. 사실 오진이라고 말씀드리기 애매한 부분은 있습니다. 피부 진료를 쭉 해오던 의료진이라면 이를 진단하지 못할리는 없다고 생각하거든요. 뼈, 근육, 침샘 자체를 구분하는 방법은 꽤 간단하고요. 단, 애초에 진단을 하는 과정이 없다면 잘못 시술할 가능성을 모두 배제할 수는 없습니다. 시술하는 원장과의 2차적인 상담과 진료가 중요한 이유이죠. 오늘은 사각턱, 영종도보톡스에 대해 알아보았습니다. 개인적으로 영종도보톡스는 잘만 사용한다면 정말 가성비 좋고, 가격 대비 효과도 뛰어난 시술이라고 생각합니다. 하지만 어디까지나 의료 시술이기에 과해서 좋을 것은 그 무엇도 없다는 점은 꼭 기억해주시기 바랍니다. 하늘도시 르네의원의 문성호 원장이었습니다.
